Financial Information
For your convenience we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is. For single visit procedures (consultations, exams and x-rays), the payment is due on the day of the visit. For multiple visit procedures (crowns, bridges, dentures), 50% of the total treatment fee is due on the day treatment begins.
Our office will be pleased to prepare dental insurance forms for you to submit to your carrier. Our office does not accept checks from insurance companies. Dental reimbursement checks will be sent directly to the patient by the patient’s insurance company.
